Freedom of Choice – Can choose any dentist. Plan pays a flat dollar amount per dental procedure based on the fee schedule for the insured’s specific region. You can also go to one of the over 73,000 access points and generally pay even less. The plan will pay the lesser of the provider’s actual charge or the amount listed on the Schedule of Covered Dental Procedures. Easy to Get - Age 19 and above – no age limit; Seniors welcome!

Deductible - Just $50 per year for each covered family member.
